The Northrop Grumman-supplied payload adapter release mechanism failed to function and the Zuma satellite re-entered along with the Falcon 9 second stage. SpaceX decided that commemorating the mission wasn't respectful of the loss suffered by the US government. The public Zuma patch was recalled by SpaceX after a few days of issue, causing an immediate scarcity amongst collectors.
Internally however the Zuma Mission was a success, with the efforts of the employees being flawless. The numbered employee patches were issued as normal and they now outnumber the blank public patches.
SpaceX changed this patched after the launch, the twill (base material) of the patch warped so before the Cargo Dragon landed a new and updated version of the patch was made.
